NEW STUDENT REGISTRATION

Thank you for your interest in joining the Cloverleaf Community!

Please click here to start your FinalForms application. (Please note: Only the custodial parent / legal guardian can officially enroll a student.)

Below is a list of required documents that will need to be uploaded to your student’s FinalForms profile:

  • Birth certificate
  • Parent / guardian ID
  • A proof of residency (dated within the last 60 days). Acceptable proofs of residency: Utility bill, real estate tax bill, bank statement, recent receipt with parent's current address on the receipt, pay stub, lease agreement or a notarized residency affidavit** (Please click HERE for an affidavit form.)
  • Divorce / custody papers (if applicable)
**If you do not own or rent your residence, a notarized residency affidavit (see link above) and one form of proof of residency with the name of the owner of the home is required.**

The registration process cannot be completed until ALL required documents are provided.

Once your student’s FinalForms have been completed and all required documents have been reviewed, your student’s registration is complete. You will receive a confirmation email along with important information to help with your transition into the district. Staff from the appropriate school building will reach out to you with important details regarding the first day of school, transportation, etc.
