Kindergarten registration for the 2025-26 school year opens Jan. 6, 2025. Students registering for the 2025-26 school year must be at least 5 years old on or before Aug. 1, 2025.

_____ Schedule your student’s Kindergarten Screening appointment. Click here
_____ Schedule an appointment with your doctor for: Immunizations Physical Form
_____ Complete your student’s Health History Form
_____ Pay the $80 Kindergarten School Fees at Kindergarten Screening. You may pay fees via check, credit card, debit card or apply for the Free and Reduced Lunch Program as of Aug. 1, 2025
_____ Attend Kindergarten State Testing Appointment (more information to come at Kindergarten screening!)

___ Parent/Guardian ID
___ 1 Form of Proof of Residency
(ie. Utility Bill, Lease/Purchase Agreement, Residency Affidavit**)
___ Divorce/Custody Paperwork (if applicable)**certified copy**

Kindergarten Screenings are April 24-25 at Cloverleaf Elementary School, 8337 Friendsville Road, Seville, Ohio 44273.
**If you do not own or rent your residence, a notarized residency affidavit and one form of proof of residency with the name of the owner of the home is required. **
