New Student Registration

Welcome to Cloverleaf Schools! The link below will connect you to our online registration forms. After you have completed all the forms, please choose a date and time to bring your required documentation from the checklist below to meet with the registration secretary and finish the registration process.
Required documentation to bring with you: 
•Birth Certificate or Valid Passport
•Immunization Records
•Proof of Residency
•Parent/Guardian Picture ID
•Divorce/Custody Papers, if applicable (must be court certified)
Below is a list of documents that will help with the scheduling process:
•Most recent IEP, ETR or 504 Plan (if applicable)
•Copy of Last Report Card (K-8) or Unofficial Transcript (9-12)
•Standardized Test Scores
If you arrive with all required documents, your appointment should take no longer than 15 minutes.
The Central Registration Office is located at Cloverleaf High School, 8525 Friendsville Road. Please come to the main front door of the building. You will sign in at the high school office and be directed to the registration office. Please click HERE for a map to Cloverleaf High School. If school is canceled due to inclement weather or for some other circumstance, the Central Registration Office will be closed. School cancelations are posted at
If you do not bring ALL required documents, you will need to schedule another appointment. The registration process cannot be completed until all required documents are brought in.
If you are living with someone, they will need to fill out a Residence Affidavit Form and have it notarized. Please attach a utility bill to the notarized Residence Affidavit and bring them to your appointment.
