What does a transition coordinator do?
The role of the transition coordinator is to help students with disabilities and their families navigate the transition from school to adulthood. This process starts when students turn 14. We utilize a backwards planning process to support students. By early planning, students can achieve their goals in employment, education and independent living.
The transition coordinator can support students and families by:
- Connecting students with adult services within Medina County (Medina County Board of DD)
- Connecting students with employability supports within the county (Opportunities for Ohioans with Disabilities)
- Arranging job shadowing opportunities
- Arranging job placements outside of the school building
- Connecting students with the Medina County Career Center
- Working with the school to provide field trips for community outings
- Supporting students with navigating the Student Accessibility Services in various colleges and universities
- Providing activities that support the student's (and their family's) future goals
- Public transportation support
- Plus much more!
