Sports physicals for students in grades 6-12 are available from 3:30 p.m. to 5 p.m. on Feb. 10 at Cloverleaf Middle School.

Cost is $10. You must call the school nurse between 9 a.m. and 2 p.m. at 330-302-0203 to make an appointment. Space is limited to 50 athletes, so call the nurse to make an appointment as soon as possible.

IMPORTANT: Page one of the OHSAA History Form must be completed, signed by a parent and brought to the physical before the student can be seen. No exceptions! Sports physical forms are available on the Athletic Web site at www.coltsathletics.org under the heading “Forms.” Payment in cash or check must be made when the athlete checks in for the physical. Checks should be made payable to “Cloverleaf Athletics.”
