The Unclaimed Funds list below shows checks Cloverleaf has issued to employees, vendors, etc., that were never cashed. If your name is on the list and you are a non-employee, just print the Unclaimed Funds Claim Form below, fill it out and return it to the Treasurer's Office. You may scan and e-mail the completed form to firstname.lastname@example.org
Cloverleaf employees must have a direct deposit form on file with the Treasurer's Office to receive unclaimed funds. If you do not have one on file, just log in to the Staff Resources page HERE
to obtain a direct deposit form and return it to the Treasurer's Office along with a copy of the Unclaimed Funds Claim Form.
